The northsidewomenswellbeinghub.com.au, ABN 45 495 144 765, operating as Northside Women’s Wellbeing Hub, are committed to protecting your privacy as a client and online visitor to our website. For the purposes of these terms and conditions, “Us”, “Our” and “We” refers to northsidewomenswellbeinghub.com.au and “You” and “Your” refers to you, the client, visitor, website user or person using our website.


We are an Australian Privacy Principle Entity (APP Entity) as defined in the Privacy Act 1988 (Cth) (the Act). We adhere to the Australian Privacy Principles when handling your personal information, together with the Health Records Act when handling personal information which includes health information. We take the protection of your personal information seriously, as well as recognise the importance of your privacy and rights to control how your personal information is collected and used. 


The following Privacy Policy has been prepared in accordance with the APP and sets out:

  • The kinds of personal information that we collect and hold
  • How we collect and hold personal information
  • The purposes for which we collect, hold, use, and disclose personal information
  • How you may access personal information about you that is held by us and seek the correction of such information
  • How you may complain about a breach of the Australian Privacy Principles and how we will deal with such a complaint
  • Whether we disclose personal information to overseas recipients


In this policy:

‘Personal information’ has the same meaning as in the Act [section 6(1) of the Act] and includes:

  • ’Information or an opinion about an identified individual, or an individual who is reasonably identifiable’


The personal information you provide us may include, amongst other things, your name, address, email address and phone number. Your personal information may also include Health information:

  • As part of delivering Our Services, we may collect or infer health and other sensitive information about you as defined by the Act.


Personal information we collect from you

We collect personal information for the purposes of determining whether we are the right service for you, arranging appointments, delivering psychological services, and communicating with you. We may collect information in the following ways:

  • Brief intake phone calls
  • Intake form (hosted by Snapforms) sent to the client by email following an appointment booking through the Cliniko website for the purpose of collecting personal information.
  • Through Cliniko for the purpose of storing your details, session notes, and any other service provision information. 
  • Through Medipass, for the purpose of processing rebates.
  • Through NovoPsych for the purposes of assessments. 
  • Through Eway, for the purpose of collecting and storing your credit card details. 
  •  Or in the course of us undertaking psychological services in consultation and treatment with you, the client. 


We collect personal information:

  • From clients – when you request an appointment, book an appointment, attend an appointment, attend our clinic, and access our website
  • From third party service providers such as Cliniko, SnapForms, Medipass, NovoPsych, and Eway, or when a service is engaged to supply services to us, provide information to us, or invoice us for the provision of services. 
  • When we receive enquiries through the Website and when the Website is accessed or used
  • When we receive enquiries via email or telephone


To provide Our Service to you, we may collect personal and health information including but not limited to the following:

  • Your name
  • Date of birth
  • Contact details (e.g., address, email address, phone number)
  • Gender
  • Credit card or debit card details
  • Emergency contact details
  • Type of appointment and times you are requesting
  • Information about your private health insurance fund, including your membership number
  • Your Medicare number and details
  • Medical reports, referrals, MHCP, and mental health history

If you do not provide us with all the information we request, we may not be able to deliver the psychological services to you. The Personal and Health information we collect enables us to arrange for the provision of psychological services, to assess the needs of clients in the provision of psychological services, and for us to optimise the quality of our service. 


Consent

By using our website, engaging us, registering your details on our Website or by email or by telephone or by using our forms embedded into our website hosted by a third party (e.g., SnapForms, Cliniko), or completing a client consent form, or otherwise engaging in our psychological services, you consent to us collecting, using, and disclosing your personal (and health) information for the purposes specified in this Privacy Policy. 


Use

We may use any personal information (excluding health information) to provide psychological services, operate the website, respond to feedback and complaints, communicate with third party service providers, and to enforce our Client Consent Form with you. 


Health Information

We also collect your Health Information when you complete our intake form, attend an appointment, and/or at any other time during the course of arranging the provision of psychological services. 


We collect your Health Information:

  • As a necessary provision of the psychological services, including recording personal and health information which is relevant to your situation for the purposes of assessment and diagnosis in your client notes
  • Only from our clients or from someone authorised by them (including caregivers)
  • Only as necessary for the provision of psychological services, or with your consent
  • If required by law (including court orders)
  • As reasonably possible, taking all stepts necessary to ensure your information remains confidential and that any persons it is disclosed to are also required to keep it confidential in accordance with the duty of care to you and the Ethical Guidelines of the Australian Psychological Society
  • You may at any time request a copy of your Health Information by emailing us at info@northsidewomenswellbeinghub.com.au (subject always to applicable law). 


Where you have concerns that the information (personal and health) we hold may be incorrect or inaccurate, we will rectify any inaccuracies through further information you provide


We comply with the Health Privacy Principles and the Ethical Guidelines of the Australian Psychological Society in the collection, use, and disclosure of all Health information, as well as to ensure to the best of our abilities that your Health Information is kept strictly confidential. 


Disclosure and sharing

We do not share your personal information with third parties without your consent in accordance with this Privacy Policy, except as required by law or as necessary to provide you with the services you request.

Data storage, security, and quality

We will take reasonable steps to protect your personal information from misuse, loss, unauthorised access, modification, or disclosure. We cannot guarantee the security of any personal information transmitted to us via the internet. Such transmission is at your risk. This is an inherent risk you assume when you use Our Services. 


Northside Women’s Wellbeing Hub uses the following servers to store your information:

  • NovoPsych (Policy can be found HERE)
  • Eway (Policy can be found HERE)


We will also take reasonable steps to make sure your personal and health information is accurate, up to date when we collect it, and complete. To assist us, please ensure you provide us with your correct details, and inform us when you believe the information, we have about you may be incorrect, inaccurate, incomplete, out of date, or misleading. We will take reasonable steps to rectify this. 


We may take steps to destroy or permanently de-identify information when it is no longer required for any purposes for which it was used or disclosed. 

Personal information may be stored electronically through third party data centres, which may be located overseas. Wherever possible, we use third parties who provide high standards of data security and storage in compliance with the Act.
